Salon Policy.
Cancellation Policy:
I understand that any appointment at Boss Lady Salon not cancelled within 24 hours of the scheduled date and time will prompt the implementation of Boss Lady Salon’s cancellation policy protocol. My card on file will be charged 25% of what the scheduled session services were valued at.
Rescheduling Policy:
I understand that after the second rescheduling of the same session, I will be required to put down a $25 non-refundable deposit to hold my session. This deposit will only be credited towards my session if it is not rescheduled again. Any future reschedules will also require an additional $25 non-refundable deposit.
Tardiness:
If I arrive 15 or more minutes after my scheduled time, I will be rescheduled, unless otherwise instructed, due to interference with the next session. My session is then within the cancellation policy protocol and my card on file will be charged 25% of what the scheduled session services were valued at.
Saved Payment Method:
I understand and agree with my payment methods being safely and securely stored on file with Boss Lady Salon’s POS system, Square. Square is PCI compliant and innovative in fraud prevention to protect your data.
Redo Policy:
If we discussed something in the consultation that I feel was not achieved, I will reach out to Boss Lady Salon to discuss the situation. In most circumstances, Boss Lady Salon is pleased to accommodate free of charge. For said complimentary service to be done, this request must be made within 7 days of my initial appointment.
I understand no refunds are allowed under any circumstance.
Product Return Policy:
I understand that a very lightly used product may be returned within 60 days of purchase. I will make sure to reach out to Boss Lady Salon with my concerns as soon as possible after using the product.
Salon Etiquette:
Discriminatory/Derogatory statements & actions are prohibited in the salon and will not be tolerated.
By reading and agreeing to this statement I have read and understood all of Boss Lady Salon’s Policies found in the Digital Welcome Packet sent to my email and stated in this memo. I agree to have my payment info saved and stored securely.
I understand that at Boss Lady Salon will have an in depth conversation speaking about all aspects of today's appointment, in which I agree to and acknowledge including price of service, and the expected physical and aesthetic results.
I hereby release and agree to hold Boss Lady Salon harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the salon, or that may otherwise arise in any way in connection with any services received from Boss Lady Salon. I understand that this release discharges Boss Lady Salon from any liability or claim that I, my heirs, or any personal representatives may have against the salon with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from Boss Lady Salon. This liability waiver and release extends to the salon together with all owners, partners, and employees.
Covid/Policy Waiver:
I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing.
I further acknowledge that Boss Lady Salon has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19, and all other communicable diseases/infections.
I further acknowledge that Boss Lady Salon can not guarantee that I will not become infected with the Coronavirus/Covid-19 or any other communicable disease/infection.
I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 or any other disease/infection may result from the actions, omissions, or negligence of myself and others, including, but not limited to, salon staff, and other salon clients and their families.
I voluntarily seek services provided by Boss Lady Salon and acknowledge that I am increasing my risk to exposure to the Coronavirus/COVID-19. I acknowledge that I must comply with all set procedures to reduce the spread while attending my appointment.
I attest that:
* I am not experiencing any symptom(s) of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell.
* I have not traveled internationally within the last 14 days.
* I do not believe I have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19.
* I have not been diagnosed with Coronavirus/Covid-19 and not yet cleared as non contagious by state or local public health authorities.
* I am following all CDC recommended guidelines as much as possible and limiting my exposure to the Coronavirus/COVID-19.
